Week 5 – Assignment: Conduct Research to Identify Barriers
Instructions
This week, you have the opportunity to conduct further research into your chosen topic area by examining barriers that might influence implementing evidence-based practice in your community. Use your research to create an annotated bibliography of your findings.
Continue your research on the topic (topic: Improving of collaboration in healthcare) you chose in Week 1 and think about barriers that could impact improvements conducted through evidence-based practice. Barriers might include lack of research, provider acceptance, lack of community support, funding, or other items you locate through additional research. Be sure to use very current sources for your annotated bibliography and include a paragraph as to why you find your chosen source of value.
Length: 5 resource annotations, not including title and reference pages
References: Include a minimum of 5 scholarly resources.
The completed assignment should demonstrate thoughtful consideration of the ideas and concepts presented in the course by providing new thoughts and insights relating directly to this topic. The content should reflect scholarly writing and current 7th edition of APA standards.
Refer to the Unit III Required Unit Resources, and review the “Example Implementation Plan,” which is provided on the Improving Patient Flow and Reducing Emergency Department Crowding: A Guide for Hospitals webpage. Use the resource as a template for developing your own implementation plan.
Identify a target population in need of a public health intervention. For example, you might decide to target low-income African American women who are overweight, and the intervention would propose an appropriate physical activity program. Another example might be targeting a sub-population with high rates of diabetes, and the intervention would be focused on changing their dietary habits. (Target Population: Blacks & Hispanics Getting Vaccinated at A Lower Rate than Whites)
Your implementation plan must have the components listed below.

In the “Problem Statement” section of the implementation plan, make certain to include at least three in-text citations (and corresponding reference entries) to support the need for the plan you are developing.
Your plan should be at least four pages in length, be double-spaced, be typed in 12-point Times New Roman font. Adhere to APA Style when constructing this assignment, including in-text citations and references for all sources that are used. Please note that no abstract is needed.
